Chapter 35 - Survivors & Dependents Assistance 

The Survivors & Dependents Assistance benefit is available to qualified spouses and children of a veteran who died or is permanently and totally disabled as a result of a service-connected disability

New and First Time Transfer Students

Students new to the University and utilizing the Survivors & Dependents Assistance benefit will need to complete/submit the following:

• VA Form 22-5490 Dependents' Application for VA Education Benefits (New Students)
• VA Form 22-5495 Dependents' Request for Change of Program or Place of Training (Transfer Students)
• Complete the Web Authorization Verification of Enrollment (WAVE) through the Department of Veteran's Affairs website.
  This form initiates the enrollment verification process through the VA
  
• SCSU VA Request for Enrollment Certification available from the Bursar's Office website.
  This form is used to provide required information to the Bursar's Office as part of the VA enrollment certification process
  

Continuing Students

Continuing students utilizing the Survivors & Dependents Assistance benefit will need to complete/submit the following each academic year:

• SCSU VA Request for Enrollment Certification available from the Bursar's Office website.
  This form is used to provide required information to the Bursar's Office as part of the VA enrollment certification process
