Preparation For Graduation: The Graduation Audit:

 

As you approach your final semester you will need to prepare for graduation. Please con­sult the information below and follow the guidelines presented for a hassle‑free gradua­tion:

 

1.         You must apply on-line for graduation. Check the University semester bulletins. The deadline is usually found in the semester calendar at the beginning of the bulletin. You must apply prior to the semester in which you intend to graduate unless otherwise indicated;

 

2.         Be sure you have completed or are in the process of completing, all of the course work indicated on your Planned Program of Graduate Study;

 

4.         Be sure that the following information is present on your University transcript:

 

a.         pass or waiver indicator for Praxis I examination (for those wishing to apply for teacher certification in any state). You do not need this to graduate, but you do need it for teacher certification endorsement in any state;

 

b.         pass indicator for the ASHA national exam (listed as Praxis II on transcript). You must have this listed on your transcript in order to graduate;

 

c.         successful completion of necessary "teacher certification requirements," (including the special education course) if they are part of your planned program;

 

d.         pass indicator for ­CMD 605 - Special Projects, or;

 

e.         successful completion of thesis indicator;

 

f.          a grade point average of 3.0 or better.

 

5.                  Be sure that you have completed or are completing all required ASHA clinical     clock hours.

 

6.            When you apply for graduation the registrar sends to the program director a  graduation sign-off sheet and an unofficial copy of your transcript. The form asks the director to confirm your eligibility for graduation. The program director will perform an audit of your program records and will respond as follows:

 

a.                  If all requirements have been met including completion of hours, the director will indicate “Eligible” with no conditions.

 

b.                  If any of the above items needed for graduation are missing, the director will indicate “Eligible when completes...” then the items that need to be completed will be listed (this will most frequently be the case as Praxis II, final course work, and your clinical hours may not yet be completed). When items are completed to the Registrar’s satisfaction, you will be permitted to graduate.

 

A copy of this form will be sent to you and a copy will be placed in your departmental file. If you are preparing to graduate during the spring or summer semester of the same year or in the fall semester of the prior year, the graduate school will send information regarding graduate commencement. You will be asked to check your graduation status with your advisor. The information in this form will serve as an indicator of your graduation status.

 

7.                  If the program director has problems with your audit, you will be contacted.

 

8.                  When you complete your clinical hours, send them to the director in an envelope. On the front of the envelope please write:

 

          a.    Working in the (insert season) (if this is the case);

       

          b.    Please send letter to the State Department of Public Health  (If you will be seeking Connecticut licensing).

 

These clinical hours will be processed with priority.

 

9.                  American Speech-Language Hearing Association application for clinical certification: if you wish to apply to ASHA enclose your completed ASHA application in the envelope you use to send in your hours. Please send pages1, 2, and the final Director Verification page only. The program director will sign the application if all requirements have been met. A copy of the application will be made for your file and the original will be sent back to you; you then include your check and send it off to ASHA. Please refer to the section on “ASHA Certificate of Clinical Competence” in this handbook for information on applying for ASHA clinical certification, where to apply, and a phone number. You do not need to complete this application in order to graduate.

 

10.             Department of Public Health license application: PLEASE BE AWARE: YOU MUST BE IN THE PROCESS OF APPLYING FOR STATE LICENSURE PRIOR TO BEGINNING EMPLOYMENT IN CT AS A SPEECH‑LANGUAGE PATHOLOGIST/ AUDIOLOGIST. Please refer to the section on "Connecticut State Licensure" in this handbook for informa­tion on applying for a license, where to apply, whom to contact, and a phone number. The director does not need to see or sign your licensing application. When all degree requirements are met the director will send a letter (the letter you requested in # 8) to the Department of Public Health so indicating. This is all that is required from the CMD Department. You do not need to make application for licensing in order to graduate.

 

11.             State Department of Education application for teacher certification. PLEASE BE AWARE: YOU MUST BE IN THE PROCESS OF APPLYING FOR TEACHER CERTIFICATION PRIOR TO BEGINNING EMPLOYMENT AS A SPEECH‑LAN­GUAGE PATHOLOGIST IN CT SCHOOL SYSTEMS. Please refer to the section on "Connecticut State Teacher Certification" in this handbook for information on applying for a teacher certifi­cate, where to apply, and a phone number. The director does not need to see or sign your application for teacher certification. However, there is a page in the application that calls for the University seal. You must send this form directly to the Registrar for signature and seal - not to the Department!! Once the Registrar receives this page they will send the director their form to verify eligibility for teacher certification. The director will sign the form if all requirements have been met. You will receive a copy of this form and a copy will be placed in your Department file. When the Registrar’s office receives the director’s signed verification they will sign and seal your application page and send it to the State Department of Education. You do not need to have this application completed in order to graduate.

 

Audit Synopsis:

 

During your final academic semester you will receive from the Department, a synopsis of information required for graduation. If you do not receive it, please contact your program advisor and obtain a copy.
