Evaluation of the Clinical Instructor

This evaluation is very important in the continued improvement of our Athletic Training Education Program. It is only through your honest and objective critique (evaluation) that we can help our Clinical Instructors become better educators, which will in turn make you a better student. You are encouraged to write specific comments, as you feel necessary. Your name will remain anonymous. Please fill out and submit one form for each clinical instructor on the pull down menu. This form will be electronically mailed to Mr. Charles Davis, Director of Athletic Trianing Education. If there is a problem with this form please contact the Athletic Training Education Department.
