HOW TO REQUEST A TAX TRANSCRIPT

1. Go to http://www.irs.gov
2. Click on Order a Return or Account Transcript under the Tools section
3. Complete the requested information:  social security number, date of birth and street address.
4. Under Type of Transcript select Return Transcript
5. Select 2011
6. Click Continue
7. A message will appear letting you know the status.
8. Once you have received the tax transcripts, sign the copies, write the student's ID number on it and submit it to the financial aid office
