Graduate Commencement 2010

Please fill out the form below if you are planning to participate in the May 2010 Graduate Commencement.  This form is to participate in the ceremony and must be submitted by December 1, 2009.  Also note that you MUST file an Application for Degree Form in order to obtain your degree and to participate in the ceremony.  The deadlines for completing the Application for Degree Form are as follows:

        Monday, November 2, 2009 for May 2010

        Friday, April 9, 2010 for August 2010
