Athletic Physical exam form for new  students

This form is to be filled out by the NEW incoming student and by a medical provider PRIOR to entering the university. 

The Athletic Form can be done at the same time as the Connecticut State University Student Health Services Form. Both must be submitted to participate in the Athletic program.

Neither of these examinations will not be done at the Granoff Health Center.

All athletic forms must be submitted to the Athletic Trainer prior to June 30, 2012.
