New Student Athletes

Athletic physical form for participation in Athletics at SCSU for new athletes:

Athletic Physical exam form for new  students

This form is to be filled out by the  incoming student and by their  healthcare provider.

The Athletic Form can be done at the same time as the Connecticut State University Student Health Services Form. Both MUST be submitted to participate in the Athletic program.

All athletic forms must be submitted to the Athletic Trainer prior to June 30, 2013.
