Women's annual visit form

This the form you need to fill out prior to your Women's Health exam. 

• It should be completed PRIOR to your visit or

• You should arrive for the visit 15 minutes prior to your scheduled appointment.

FORM TO RELEASE Medical INFORMATION TO  YOURSELF OR ANOTHER AGENCY

 This form must be filled out, signed, and sent to Granoff Student Health Services via

After this is form is received we will FAX or MAIL your information as directed on the form

Form to Obtain medical information from another provider's office 

This form must be filled out, signed and sent to Granoff Student Health Services via

After this is form is received we will FAX or MAIL your information as directed on the form to obtain the medical records or immunization information listed

Form to apply for a special Parking permit

This is the form  needed to apply for short term special parking on Campus.

This is only to be used in the event of a short term medical problem.  If a longer parking permit is needed, you must obtain a State of Connecticut handicapped parking permit

form for Special Housing Request

This is the form needed to apply for special housing on campus.

Form to request a waiver for required immunizations 

This form must be presented if you are declining to have the required immunizations because of:

• Religious objection
• Medical reason ( including pregnancy)
