NOTE: In long term mentoring situations, evaluation should take place after 30 days, after 90 days, and at six months. In short term mentoring situations, evaluation should take place monthly or weekly, depending on the intensity of the contact and the set goals and objectives. Revised mentoring goals and objectives can be submitted on the Mentoring Plan Form.
