NOTE: A Mentoring Plan or Agreement is meant to establish expectations so that neither the mentor nor the mentee will be disappointed. The Mentoring Agreement should list three key points or goals which the mentor and mentee agree to work toward. Mentoring Agreements need to be evaluated periodically and goals and objectives adjusted. Evaluation of the mentoring plan can be submitted on the Mentoring Evaluation Form. Revised mentoring goals and objectives can be submitted on the Mentoring Plan Form.
